Dog Liability Release Form

This agreement is between Colorado Bed & Barn, its Owners Lorrie & Al Todd, and Colorado Bed & Barn, guests _____________________________________.

In making their reservation guests have indicated they will be accompanied by their pet dog(s) named:_______________________________

The guest/pet owner represents and warrants that the dog is healthy, in good condition, and has not been exposed to any infectious or contagious disease, and has a currently valid rabies inoculation.  Guest/pet owner agrees that if immediate or emergency care is needed and provided, the guest/pet owner shall be solely responsible for the cost of any and all such care.

The guests assume and bear all the risk of loss, injury and damage of any kind to their pet dog while at or on the property of Colorado Bed & Barn, whether caused through the negligence of the guest, other guests, or lodge owners or employees, or lodge animals and shall hold the lodge harmless for such claims.

The guest/pet owner assumes and shall bear all risk of loss, injury and damage of any kind or nature to any person or property of the Todds, Colorado Bed & Barn, and other guests caused by the dog, the actions or inaction by the guest/pet owner and agrees to indemnify and defend Colorado Bed & Barn and hold it harmless for such claims.

This instrument contains the entire agreement of the parties.  This Agreement shall be governed by and construed in accordance with the laws of the State of Colorado.
